"At FaleOfaz, our mission has always been rooted in one thing — genuine care for the individuals and families we serve."

FaleOfaz was founded by three brothers — Jose Fesili, Legrand Z Wolfgramm, and Manisela Wolfgramm — who grew up right here in West Valley City, Utah. Rooted in faith, family, and community, they built FaleOfaz to be a place where Salt Lake County residents with disabilities are truly seen, celebrated, and supported.

Group Home Certification & Licensed Home — Up to 5 Clients

FaleOfaz holds both a Group Home certification and a Licensed Home certification through Utah's Office of Licensing. Our licensed home is approved to serve up to 5 clients, providing a true home environment with 24/7 structured support, safety, and full DSPD compliance.

RHS Residential Habilitation Supports

Skilled residential assistance in a certified group home setting, supporting each person to live as independently and self-determined a life as possible — fully integrated in their Salt Lake County community.

DSI Day Supports — Individual

All services are provided in integrated, community-based settings that ensure privacy, dignity, autonomy, and individual rights per the 2023 Utah HCBS Final Rule.

Background Check Verified Staff

All staff maintain current eligible background check determinations through Utah's DACS system before working unsupervised with any client.

Person-Centered Planning

Each client receives an individualized Person-Centered Support Plan (PCSP) aligned with their unique goals, needs, and personal preferences.

Health, Safety & Emergency Ready

All sites maintain first aid kits, fire extinguishers (2A:10B:C), smoke & CO detectors on every level, and documented emergency response plans.
