FaleOfaz was founded by three brothers — Jose Fesili, Legrand Z Wolfgramm, and Manisela Wolfgramm — who grew up right here in West Valley City, Utah. Rooted in faith, family, and community, they built FaleOfaz to be a place where Salt Lake County residents with disabilities are truly seen, celebrated, and supported.

Skilled residential assistance in a certified group home setting, supporting each person to live as independently and self-determined a life as possible — fully integrated in their Salt Lake County community.
One-on-one day support providing safe supervision, daily skill-building, and community participation. Designed to prevent isolation and empower each person's unique goals and choices.
Group-based day programming (averaging 6 hours/day) providing structured support, social engagement, and community involvement in a safe, enriching environment.
Coordinated, reliable transportation to and from day support and supported employment programs — removing barriers and keeping every appointment on track.
Supervision and support provided during times when a resident is unable to attend their day program — ensuring continuous, uninterrupted care and daily structure at home.
Support to help individuals manage their personal finances effectively — building real-life money management skills and fostering greater financial independence.

FaleOfaz operates under full compliance with Utah's Division of Services for People with Disabilities (DSPD), DHHS, and federal HCBS regulations.
Certified by Utah's Office of Licensing with both a Group Home certification and a Licensed Home certification — approved to serve up to 5 clients.
All services are provided in integrated, community-based settings that ensure privacy, dignity, autonomy, and individual rights per the 2023 Utah HCBS Final Rule.
All staff maintain current eligible background check determinations through Utah's DACS system before working unsupervised with any client.
Each client receives an individualized Person-Centered Support Plan (PCSP) aligned with their unique goals, needs, and personal preferences.

All sites maintain first aid kits, fire extinguishers (2A:10B:C), smoke & CO detectors on every level, and documented emergency response plans.
